When a patient id card is created, the patient is required to provide the card number and the name of the card-carrying institution. The patient is also required to provide the unique identifier for the card. The identifier is a 2-digit number that is unique to the patient. By printing the member's name and gender on the card, members can more easily remember who is front-and-center when posing for a photo or in a group photo. The card also doubles as an identity card if a member is lost or forgotten. If your cardholder id card is lost or stolen, you should report the incident to your insurance protector as soon as possible. There may be punitive damages available if your cardholder id card is lost or stolen. They may have assigned a student procurer to look for them, or they may have a rosters system in place that helps you find your id card student counterpart. Once you have provided all of the necessary documentation, you will need to provide a scan of your identification card and social security card to DSWD. This will usually take about two hours.
